Waiver Terms and Conditions
Participation of any sport can be dangerous and it is impossible to ensure the safety of all participants. Soccer demands
cardiovascular fitness, coordination, and agility. Although many risks can be avoided, I understand that not all risks can be and hereby
fully and forever release My Soccer Academy their employees or any of its representatives, coaches, assistants, agents or assigns,
including the owners of fields and facilities utilized for programs, against any claim by or on behalf of the registrant/participant as a
result of the registrant’s participation in the programs.
I understand that I am fully responsible for the transportation from and to practices, games and any activity related to My Soccer
Academy. Our staff is not authorized to provide these types of services mentioned above.
I certify and affirm that: (I) my child is not currently under the care of a physician for an injury or illness that would prevent his/her
safe participation in the soccer programs. (II) my child has no history of fainting or other problems related to strenuous exercise; and
(III) my child is in good health and there’s no reason that he/she cannot safely participate in physical activity.
In consideration for accepting my child into My Soccer Academy, I do hereby agree that I am and shall be responsible for all costs
associated with any injury or loss that may result with my child as a result of his/her participation in any activities at the field. I
further understand that in the event of a medical emergency, My Soccer Academy will call Emergency Medical Services to render
assistance and that I (parent/guardian) will be financially responsible for any expenses involved.
I hereby give consent to have my child photographed and/or video-taped or audio-taped during activities, and I agree that the
images obtained may be used for educational and public relations purposes by My Soccer Academy. I fully renounce all claims upon
My Soccer Academy for reimbursement for the use of this material.
I understand that My Soccer Academy is not responsible for any personal belongings, so please we encourage you not to bring any
valuables to class.
WAIVER OF LIABILITY: In consideration of being permitted to play soccer, on behalf of myself, my family, my heirs, and my assigns,
I hereby release My Soccer Academy, its agents, and its employees from liability for injury, loss or death to the participant while
using any facility or equipment or in any way associated with participating in the activity of soccer now or in the future, resulting
from the ordinary negligence of My Soccer Academy, its agents and employees.
